Join the Eyre and Far North Local Health Network as a Community Support Worker and be part of a transformative care model that puts consumers at the heart of decision-making.In this rewarding role you will support individuals who are aged frail or disabled by providing essential services such as personal care meal preparation cleaning transport and social support.
You will work directly with consumers to help them maintain independence make choices about their care through our Consumer Directed Care model
- Provide personal care cleaning meal preparation and transport
- Work with consumers families and health professionals to create maintain personalised care plans
- Offer emotional social support to improve well-being independence
