physical,psychological, and social needs taking into consideration 
privacy, dignity,choice and informed consent.
Carryout risk assessment of patients developing individualise care plans,
implementing, and evaluating them.
Maintains professional and ethical standards and set a high standard of 
patient care.
Undertake all aspects of medicine Management in line with legal 
professional and regulatory requirements.
Ensure proper positioning of patients, turn bed-ridden patients two hourly
and care for pressure areas to prevent pressure ulcers.
Performs accurate admission and discharge support 
Promote the well-being of patients by implementing the highest quality of
care possible.
Encourage independence by helping patients to develop and strengthen 
skills allowing them to engage in more self care.
Manage a full case load of patients transitioning from rehabilitation 
centers to life at home.
Perform comprehensive assessments, including vital signs, physical 
examinations, and medical history reviews.
Administer medications and treatments according to physician orders 
and nursing protocols.
Monitor patient conditions and report any changes or concerns to the 
healthcare team.
Collaborate with interdisciplinary teams to develop and implement 
individualized care plans.
Educate patients and families about their conditions, medications, and 
self-care techniques.
